Instructions for Request Banner Access Form

Follow these instructions to fill out the Request Banner Accses Form.

1.Please select whether this form is for a new employee or an existing employee. Enter the employee's information in the fields below the check boxes.

If an employee is NEW and replacing an existing or previous employee:

2. Enter the SID number of the employee being replaced

3. Enter the Full Name of the employee being replaced.

4. Check this box if an employee needs access to enter time for other employees in their department. MUST ENTER TIMESHEET ORGS that this employee will be entering time for. If you do not know the Orgs, please contact Payroll x4051 to get the Orgs prior to submitting this form.

5. Check this box if an employee needs access to be a Proxy to approve time. MUST ENTER TIMESHEET ORGS that this employee will be approving time for. If you do not know the Orgs, please contact Payroll x4051 to get the Orgs prior to submitting this form.

IF AN EMPLOYEE NEEDS ACCESS TO A NEW FORM:

6. List the form(s) that the employee needs access to.

7. List the type of access (Query or Modify) that the employee should have.

IF AN EMPLOYEE DOES NOT NEED ACCESS TO A CURRENT FORM OR NEEDS THE ACCESS CHANGED:

8. List the form(s) to remove, list the form where the access needs to change.

9. List the type of access (Query or Modify) that the employee should have.

10. Please make any notes or special instructions that are needed on this request.

11. After FULLY completing form, type your signature.

12. Remember to click ORDER NOW icon at the upper right hand corner of the form.
